The Ward 24 initiative demonstrates the GP Federation’s commitment to enhancing patient care and improving hospital efficiency. Working with consultants within Fairfield General Hospital, the focused care of frail patients has resulted in smoother transitions for patients returning to community-based care and reduced the risk of re-admission. The initiative has also improved discharge summaries and strengthened collaborative working between primary and secondary care.
