Care Co-ordinators

Our care co-ordinators work with patients, including the frail/elderly and those with long-term conditions, to provide co-ordinated care and support across health and care services.

Care co-ordinators review patients’ needs, create personalised care and support plans, and help patients access the services and support they require.

Working closely with GPs, they ensure that patients and their carers receive the right support at the right time, and that any changes in needs are identified and addressed.
