Our patient population is at the heart of all we do.

We work with our GP practices to deliver a wealth of health and wellbeing services tailored to the needs of our patients. This includes providing additional evening and weekend GP appointments, home visits, clinical assessments, wellbeing support, specialised clinics and services from expert pharmacists, paramedics, physiotherapists, social prescribers, care co-ordinators and mental health practitioners.

We are committed to ensuring patients receive high quality, community based, personalised healthcare that meets NHS standards and guidance.
